Jour après jour, les algorithmes d'intelligence artificielle se perfectionnent et entrent dans le plus intime de nos vies. Mais les patients sont-ils prêts à accepter leur généralisation dans le domaine de la santé ? Une étude de l'Assistance publique Hôpitaux de Paris tente d'y répondre.
L'intelligence artificielle (IA) semble avoir un boulevard dans le domaine de la santé. Mais concrètement, est-ce que les patients suivront ? Pour s'en assurer, trois chercheurs de l'Assistance publique Hôpitaux de Paris (AP-HP) ont interrogé de mai à juin 2018, près de 2.000 patients français atteints d'une maladie chronique (qui entraîne plus de six mois de traitements).
Ils dévoilent les résultats dans une étude publiée la semaine dernière dans la revue « npj Digital Medicine ». Pour la plupart des patients, l'intelligence artificielle doit aider les cliniciens à faire leurs diagnostics, mais les décisions et recommandations doivent rester en dernier ressort l'oeuvre d'un humain.
Des progrès fulgurants
Aujourd'hui, l'intelligence artificielle peut surpasser les humains dans l'analyse des électrocardiogrammes, voire dans la prédiction du développement des cancers . Mais de l'algorithme de suivi de santé, au robot qui pose des questions à la place du médecin, l'IA dans le domaine de la médecine a plusieurs déclinaisons. Et les patients ne sont pas prêts à avaler la pilule pour toutes les situations.
Maladies de l'oeil : l'intelligence artificielle meilleure que les médecins ?
C'est pourquoi, les chercheurs ont interrogé les participants à l'étude sur quatre dispositifs différents qui existent déjà, ou sont sur le point de se développer : la détection des cancers de la peau par analyse de photographies, des capteurs prêts-à-porter pour surveiller les développements d'une maladie, une chemise intelligente pour guider un suivi plutôt qu'un kinésithérapeute, ou encore un chatbot avec qui discuter pour aider à diagnostiquer les problèmes.

Plusieurs profils de patients se dégagent. Deux sur cinq seraient prêts à l'usage de l'IA pour tous les dispositifs présentés, mais à la condition que cela soit contrôlé par un humain en bout de chaîne. C'est le cas notamment pour la détection de cancer par photographie.
A l'inverse, le chatbot, dispositif qui permet de poser toutes les questions de santé qu'on a en tête, et à tout moment, est l'outil qui suscite le moins de craintes. Car les Français sont déjà nombreux à parler de leur santé en ligne .
Un profil sur cinq de l'étude irait plus loin : jusqu'à accepter un certain niveau d'automatisation sans y mettre de condition.

En conclusion, près de la moitié des patients voient ainsi positivement ces technologies arriver dans le domaine de la santé. Dans la liste des avantages appréciés : un meilleur suivi, une plus grande réactivité, et aussi un traitement médical moins lourd.
En revanche, un volant de 11 % y sont frontalement opposés, y voyant un grand danger. En premier lieu, ils s'inquiètent que l'IA remplace de façon inappropriée les humains pour leurs soins.
Deuxième crainte : que les données de santé privées soient hackées, ou que des soignants et compagnies d'assurances en abusent.
